1. Eligibility

• Open to legal residents of Louisiana, aged 18 years or older at the time of nomination.
• Employees of Parish Construction and Roofing, their immediate family members, and any affiliated companies or contractors are not eligible to be nominated or to submit nominations.

2. How to Nominate

To participate, nominators must:

• Submit a nomination form explaining why the nominee deserves the prize package.
• Affirm that all information provided is truthful, accurate, and submitted in good faith.
• Confirm that they have the nominee’s permission to share their story and personal details.

Each nominator may submit one (1) nomination.

Nomination submissions will open Monday, November 3, 2025, and will close Friday, November 21, 2025.

Parish Construction & Roofing reserves the right to verify the authenticity of any nomination, including but not limited to:

• Requesting additional information or documentation from the nominator or nominee
• Contact the nominee directly to confirm details
• Disqualifying any nomination found to contain false, misleading, or unverifiable information

Any attempt to manipulate the nomination process, impersonate another individual, or submit fictitious entries will result in immediate disqualification and may be reported to appropriate authorities if deemed fraudulent.

3. Selection & Notification

A total of twelve (12) winners will be selected based on the nomination submissions received.

Nominations will be reviewed by a selection committee from Parish Construction & Roofing. The committee will evaluate entries based on factors such as:

• The nominee’s story or need
• Their community impact or service
• The authenticity and completeness of the nomination

The selection process is conducted without regard to race, color, creed, religion, sex, gender identity, sexual orientation, national origin, disability, age, marital or familial status, income level, or any other protected classification, in full compliance with applicable federal and state anti-discrimination laws.

Winners will be selected on Tuesday, November 25, 2025, and notified by phone within three (3) business days. If a selected winner cannot be reached or declines the prize within three (3) days of initial contact, an alternate winner may be selected at the discretion of the committee.

Parish Construction & Roofing may attempt notification via alternate contact methods (e.g., email or social media) if phone contact is unsuccessful, but is not obligated to do so.

Prior to receiving any prize, each selected winner must:

• Provide a valid government-issued photo ID
• Complete any required IRS documentation (e.g., Form W-9)
• Sign and return a Winner Agreement & Release Form

Failure to complete these steps prior to the designated prize pickup date may result in forfeiture of the prize, and an alternate winner may be selected.

All decisions made by the selection committee are final and not subject to appeal.

4. Prize Details

• Each of the Twelve (12) winners will receive a prize package consisting of:
  • $1,000 check from Parish Construction & Roofing
  • $1,000 check from Duplessis Builders
  • Food Donation Voucher from Chris’s Specialty Foods
  • Shoe Donation Voucher from Rukus
• PARISH CONSTRUCTION & ROOFING $1,000 CHECK: Each winner will receive a one-time check in the amount of $1,000 issued by Parish Construction & Roofing as part of the 12 Days of Christmas Giveaway. The check has no cash equivalent beyond the stated amount, is non-transferable, and may not be exchanged or substituted for other items, services, or compensation. The prize is awarded as a charitable giveaway and does not constitute wages, compensation, or payment for services rendered. Winners must provide a valid government-issued photo ID and complete any required IRS documentation (e.g., Form W-9) before receiving the check. Parish Construction & Roofing reserves the right to verify eligibility prior to issuing the prize and to report the value of the award to the IRS as required by law if the total value equals or exceeds $600. Winners are responsible for any and all federal, state, or local taxes associated with the prize.
• DUPLESSIS BUILDERS $1,000 CHECK: Each winner will receive a one-time check in the amount of $1,000 issued by Duplessis Builders as part of the 12 Days of Christmas Giveaway. The check has no cash equivalent beyond the stated amount, is non-transferable, and may not be exchanged or substituted for other items, services, or compensation. The prize is awarded as a charitable giveaway and does not constitute wages, compensation, or payment for services rendered. Winners must provide a valid government-issued photo ID and complete any required IRS documentation (e.g., Form W-9) before receiving the check. Duplessis Builders reserves the right to verify eligibility prior to issuing the prize and to report the value of the award to the IRS as required by law if the total value equals or exceeds $600. Winners are responsible for any and all federal, state, or local taxes associated with the prize.
• RUKUS SHOE VOUCHER: Voucher valid for one (1) pair of shoes/per person living full time at a residence as part of a charitable donation program. The manager retains full discretion regarding brand, style, size, and inventory availability. Recipient acknowledges that the voucher has no cash equivalent, is non-transferable, and cannot be redeemed for credit, exchanges, or alternative products or compensation. Parish Construction and Roofing and participating partners assume no liability for sizing issues, product preferences, or availability. Must be redeemed within 90 days of issuance. Limit one voucher per household.
• CHRIS’S SPECIALTY FOODS FOOD VOUCHER: Voucher valid for a food donation package as part of a charitable program. The manager retains full discretion regarding the type, brand, quantity, and availability of included items. Recipient acknowledges that this voucher has no cash equivalent, is non-transferable, and cannot be exchanged or substituted for different items or compensation. Parish Construction and Roofing and affiliated partners assume no responsibility for dietary restrictions, allergies, or food preferences. Voucher must be redeemed within 90 days of issuance. The food donation may include a selection of nonperishable items, fresh goods, or gift cards.
• The prize is non-transferable.
• The prize package cannot be substituted, assigned, or exchanged for any other items, services, cash value, or compensation beyond the exact items and stated values provided

Prize Pickup Requirement:

• All prizes must be claimed and picked up in person at the time, date, and location designated by Parish Construction & Roofing. Winners will be notified of the pickup details at the time of selection.
• Failure to appear for prize pickup on the scheduled date and time may result in forfeiture of the prize, at Parish Construction & Roofing’s sole discretion, and an alternate winner may be selected.
• Parish Construction & Roofing reserves the right to verify the winner’s identity and require presentation of valid photo identification at the time of pickup. Prizes will not be mailed, shipped, or delivered, unless expressly stated otherwise in writing by Parish Construction & Roofing.

Food Allergy Disclaimer:

• Parish Construction and Roofing & affiliates are not responsible for any food allergies, sensitivities, or adverse reactions that may occur from consumption or use of the food donation.
• The winner assumes all responsibility for reviewing ingredients and managing any allergy or dietary concerns.

5. Additional Terms

• By submitting a nomination, nominators and nominees agree to allow Parish Construction and Roofing, Duplessis Builders, Chris’s Specialty Foods, & Rukus to use their names, cities, and photos for promotional purposes, including social media and marketing materials.
• By providing contact information, you consent to receive future promotional communications, updates, and offers.
• Parish Construction and Roofing reserves the right to modify, suspend, or terminate the contest at any time due to circumstances beyond its control.
• By participating, nominators and nominees release and hold harmless Parish Construction and Roofing and its affiliates from any claims or liabilities related to the giveaway or prize.

Winner Agreement & Release

• Publicity Release: By accepting the prize, the winner grants Parish Construction and Roofing and its affiliated partners the irrevocable, worldwide, royalty-free right and permission to use the winner’s name, likeness, biographical information, voice, statements, photographs, video recordings, and any materials or content provided by the winner (including images taken during prize delivery or fulfillment) for advertising, publicity, promotional, or other commercial purposes, in any media now known or hereafter developed, without additional compensation, notification, or approval, except where prohibited by law.

6. Compliance & Disqualification

• Multiple nominations for the same person are allowed but will be reviewed as one entry.
• Any attempt to submit fraudulent or misleading information will result in disqualification.
• Nominations for individuals who do not meet eligibility requirements will be removed from consideration.

Winner Verification and Release Requirement:

• Prior to receiving any prize, each selected winner will be required to complete, sign, and return a Winner Agreement & Release Form as a condition of prize acceptance.
• This form must be completed at the office of Parish Construction & Roofing, Address: 17830 Lavo Ln. Baton Rouge, LA 70810.

7. Tax Reporting & Obligations

• All federal, state, and local taxes associated with the receipt or use of any prize are the sole responsibility of the winner.
• Winners may be required to complete and submit an IRS Form W-9 (Request for Taxpayer Identification Number and Certification) before receiving any prize.
• If the total value of the prize(s) awarded to any individual equals or exceeds $600, Parish Construction & Roofing and affiliates will report the value of the prize to the Internal Revenue Service (IRS) using Form 1099-MISC in accordance with U.S. tax law.
• Winners are responsible for consulting their own tax advisors regarding the tax implications of accepting a prize.
• Parish Construction & Roofing and its partners make no representations or warranties regarding tax treatment and will not provide any tax advice.
